Thanks to our fiscal sponsor, Creative Visions, we are able to provide a receipt for your taxable donation. All tangible in-kind donations are tax-deductible. Fill out this form to the best of your knowledge. Include the fair market value of the items you are donating or leave that blank if you don’t know and we will research that for you.

How It Works

  • DESCRIPTION: Be descriptive – the more we know about each item the easier it will be for us to confirm your fair market value.
  • ESTIMATED FAIR MARKET VALUE: Include an estimated fair market value and how the price was determined (actual market value, thrift shop value, appraisal, other).
  • SUBMIT: If you do not have all the information now – don’t worry! Use the “Save and Continue Later” option to come back and finish at a later time.
